CURSO: SEGURIDAD DE PROCESOS - Lessons Learn from Badak LNG Plant 1983 Incident

Badak LNG Plant located at Bontang, East Kalimantan, Indonesia. It operates 8 LNG trains with 6 LNG Storage Tanks and 3 LNG Loading Docks. In 2000 - 2001 Badak LNG delivered 22 million tons per year or almost 411 cargoes per year. Success of Badak LNG Plant operation cannot be achieved without taking a good lesson learned from a big incident in 1983. Train-B Main Cryogenic Heat Exchanger was exploded in April 1983 during defrosting activity for restart-up following a scheduled shutdown. Two main root cause were identified: 1) No Formal Procedures. 2) Risk of high pressure - low pressure interface did not well recognized. After the incident, Badak LNG Plant insist all critical activities must be done in accordance to an approved procedure which has been reviewed by competent personnel and management. They hired 60 fresh graduate engineers in 3 batches and put them in a structured training program.
